The /dev//* entries _were_ created during lvm setup so the fact that they went awol points to udev. I wanted to have /usr on a (unencrypted) lvm volume. However, because cryptsetup must run before lvm to support encrypted volumes, there is a problem. It is working now only because I mount an lvm volume over a complete /usr tree.
